How do I schedule an appointment?
Please call us at 480-809-6513. When you call to schedule an appointment with one of our registered dietitians, we will ask for your insurance so we can call and check your nutrition benefits. We will also ask you to have your doctor send a referral since most insurances require us to have a doctor’s note with your specific health needs.
My insurance doesn’t require me to have a referral. Why do I need one to schedule?
Since insurances require a diagnosis to cover nutritional counseling and we can’t diagnose, we need to have a referral to have on file in order to bill insurance.
What should I bring to my appointment?
Please bring a list of medications, labs, insurance card, driver’s license, and any other doctor’s notes that you have that would help us better assist you in managing your diet.
How long are the appointments?
The initial appointment is typically 1 hour depending on your needs. Follow-up appointments are 30-45 min depending on your nutrition coaching needs.
What can I expect at my appointment?
The initial appointment includes a full assessment and a plan to work on your health needs. Follow-ups include making adjustments to your nutrition plan and finding solutions to your barriers.
Do you offer online appointments?
Yes, we offer an online nutrition program. Fill out the application and we will discuss to see if you are a good fit for the online nutrition program!
Where do you offer nutrition and wellness services?
We offer in-person nutrition and wellness services in our office located in Mesa, AZ. We also offer telehealth services to clients in Colorado, Utah, and Wyoming.
Do you offer services for eating disorders?
Yes, we offer outpatient eating disorder treatment. Whether you are just starting your journey or have been living in recovery for many years, our caring Registered Dietitian will walk alongside you on your nutrition journey.
